Case Management

As a Case Manager myself and with extensive experience working with Traumatic Brain injury clients, I understand the importance of providing not only a high-quality service but detailed and robust reports.

All of the team are CSP and HCPC registered and deliver a high standard of rehabilitation. As the Director of Neuro-Rehab @ Home, I will complete the assessments and reports of case management clients personally. This will include SMART goal setting, with up-front and transparent costings.

The team are there to ensure sustainability and continuity of treatment should there be any unexpected absence.  Case Managers and the broader Multi-Disciplinary Team will have direct  and prompt contact with therapists to ensure optimal delivery of care. 

At Neuro-Rehab @ home we understand that every case is individual, and we are keen to discuss the needs of the client. We can design a bespoke package, delivered in the client’s home and local community around their needs.
